Mentorship onboarding form
If you fill in the onboarding form, I receive your name, e-mail, phone number, role, field of work and the answers you write (level of familiarity with AI, tools used, challenges and goals). I use this data only to organize the mentoring or the training and to contact you. The responses arrive by e-mail to me and to one member of the support team; you receive a short confirmation at the address you provided. Submission is handled by a transactional e-mail provider (Resend). I do not sell or share this data with third parties for any other purpose.
Do not enter patient data in the form fields.
Visit statistics
The website uses Google Tag Manager/Google Analytics to measure visits (pages accessed, approximate origin, device type). This may use cookies and sends browsing data and your IP address to Google. You can block this tracking in your browser settings or with blocking extensions.
Third-party services loaded by the pages
- Google Fonts and Font Awesome (fonts and icons): your browser contacts the servers of Google and Cloudflare, which receive your IP address.
- Tailwind CSS (styling on some pages) is loaded from a content delivery network.
- The home page displays the latest posts of the Substack newsletter through the rss2json service.
- Links to WhatsApp, LinkedIn, Instagram and Substack lead to websites with their own policies.
Clinical tools
The calculations of the clinical tools happen in your browser: the values you type are not sent to any server of this website. Some tools store the answers, the date of birth and, in some cases, the name you provide in the browser's local storage, so you can resume your work. This data stays only in that browser and is automatically erased after 12 hours; you can also erase it whenever you want through the “Erase data from this browser” link, in the tools' footer. On a shared computer, erase the data when you finish. There is no need to provide the patient's name to use the tools.
Legal basis and retention
Form data is processed based on your consent and on procedures preliminary to a contract you request (Brazilian LGPD, art. 7, I and V). Data is kept for as long as necessary for the contact and the provision of the service; you may request deletion at any time.
Your rights
Under the Brazilian General Data Protection Law (Law 13.709/2018, art. 18), you may request confirmation of processing, access, correction, anonymization or deletion of the data, information about sharing and withdrawal of consent. Write to aydamari@gmail.com; the reply will be given within a reasonable time. You may also resort to the Brazilian National Data Protection Authority (ANPD).
Security
The website uses an encrypted connection (HTTPS) and access to restricted areas is protected. No system is fully immune to failures; in case of an incident that may affect your data, you will be informed as required by law.
